Who in the Bush Administration decided what names made the " hit list "?

Since we all know that their definition of " terrorist " and " terrorism " was rather over-reaching, I am sure that this list also contained mistakes.

The rationale(?) behind how these lists were created could be getting to the heart of this matter, but I suspect that it goes beyond that.
