Default A Brief Question

I was back in the states recently, and while passing the standard border bureaucracy (surprisingly painless this time) I noticed something interesting.

All the signs, papers, forms read "United States". Nothing wrong with this, but unless my memory serves me wrong, your august country is called the "United States of America". I was just surprised that even the official forms did not use this name.

Is the full name used anywhere? For mostly the US or United States is used, at least where I saw it.

Any insight?

PS: The entrance forms still crack me up, every time. Have you, or your family, ever participated in the Nazi persecution during the years 1933-1945, or been a member of the communist party?
