There you go. Sally is really not delusional. She knows that she still has a penis and that the surgery for removal is in two months. . She knows it hurts when she pees and needs to be examined and that the treatment requires that the penis be referred to in the fax. The doctor is well aware of the previous diagnosis, the current problem, the medications and the history, and why the patient with the penis is being called 'she'.

That is how you do this. You are trying to create problems that do not exist.
