That looks artificial don't believe its real, its one of those medical malpractices like seen in plastic surgery where one buttock is more droopy than the other. it looks like it could be filled with silicone
The anatomy of it does not look functional, the amount of blood taken from his legs to fill that will leave him anemic and possibly pass out from excitment.
