So first of all, the entire point of accusing someone of a "derangement syndrome" is to give yourself an excuse to ignore the underlying validity of the criticism presented. That being said, no honest person could take her use of "run train" as anything but completely metaphorical... nobody. On the other hand... That is a lot more ambiguous.
